A running toilet wastes water quietly. A failed supply stop empties into the floor. The difference between them is whether anybody has checked the stops.
Bathrooms contain more connections per square foot than anywhere else in a building, and most of them are hidden behind tile, in a vanity or under a floor. That is why bathroom leaks are so often discovered downstairs rather than upstairs, by which point the repair is a plumbing job plus a ceiling.
The components that fail are entirely predictable. Supply stops seize and then weep when somebody finally turns them. Flexible connectors reach the end of their life and split. Fill valves and flappers wear until the toilet runs continuously. Cartridges in tub and shower valves wear out and either drip or refuse to shut off. None of that is exotic and all of it is cheaper to address before it fails.
Supply stops that have not been operated in years, braided connectors past their service life, wax rings that have compressed and allowed a toilet to rock, shower pan and tile grout that has stopped being a water barrier, and overflow assemblies on tubs that leak only when the tub is filled above a certain point.
Four approaches to locating a leak that is producing a stain but no visible source.
The first step is establishing when it leaks. A stain that grows only after somebody showers points at the shower: the pan, the grout, the valve body or the drain connection. A stain that grows regardless of use points at a supply line under constant pressure, which is more urgent because it never stops. A stain that appears only when the tub is filled deep points at the overflow assembly, which is a specific and very common fault.
Second is isolation testing. Shutting individual supply stops and watching whether the leak continues narrows it down quickly. If the leak persists with every fixture isolated, it is upstream of the stops. If it stops when one particular fixture is isolated, the search has just become much smaller.
Third is targeted access. Where the source is narrowed to a small area, a modest inspection opening is far better than a large exploratory one, and it is agreed with you before anything is cut. An access panel left in place afterwards is frequently the sensible outcome, particularly behind a shower valve that will need attention again one day.
Fourth is moisture measurement rather than guesswork about the extent. A meter across the affected area shows how far the water has traveled, which determines what actually needs drying and replacing. Stains spread further than water does, and assuming the wet area matches the visible mark leads to both over and under repair.

A supply stop is the small valve beneath a fixture that lets you isolate it. Most are installed once and never operated again, and the packing inside dries out over the years. The first time anybody tries to close one, typically during an emergency or a fixture replacement, it either refuses to turn or turns and then weeps around the stem. Multi turn stops are particularly prone to this, and quarter turn ball type stops are considerably more reliable.
Braided flexible connectors are the other. They are convenient, they are used everywhere, and they have a service life. The braid protects against burst but the inner tube ages, and connectors that are decades old are a genuine risk, particularly on a toilet supply where a failure releases water continuously until somebody notices. A failed connector on an upper floor while a household is away is one of the more damaging things that happens to buildings.
Both are cheap to replace and both get checked on any bathroom visit, whatever the original call was about. Replacing a twenty year old connector and a seized stop while somebody is already under the vanity costs very little, and it is the single most cost effective preventive item in a bathroom.

That depends entirely on the source, which is why locating it properly comes first. A pressurised supply leak runs continuously and does damage quickly. A shower pan or grout failure only leaks when the shower is used, and an overflow fault only when the tub is filled deep.
Shut off the water to the bathroom if the stain is growing while nothing is being used, since that indicates a supply leak. Otherwise avoid using the bathroom until it has been looked at, which limits how much more water goes in.
Considerably more than people expect, because it runs continuously and silently once the household stops noticing it. A worn flapper passing water into the bowl around the clock adds up across a billing period in a way that is genuinely visible.
The parts are inexpensive and the repair is quick. It is one of the few plumbing repairs that reliably pays for itself.
Not always, and the diagnosis determines it. Many shower leaks are at the valve body, the drain connection or the overflow, all of which can be reached from behind or below in a lot of buildings.
Where the failure is in the pan or the waterproofing behind the tile, that is a larger job and it gets explained honestly rather than approached optimistically. Any access is agreed with you before it is made.
Several possibilities, and they are distinguished by whether it affects hot only, cold only or both, and whether it affects one fixture or all of them. One fixture affected on both hot and cold usually means an aerator or a cartridge. Hot only across the building points at the water heater or its outlet.
Whole building low pressure on both is a supply side question, including whether a pressure reducing valve has failed. Measuring the static pressure settles it quickly.
They are, when they are within their service life. The braid is there to contain a failure of the inner tube, and it works. What is not safe is leaving one in service for decades, because the inner tube ages regardless of how the outside looks.

Frequently repairable, and it is worth asking. Cartridges, seats, stems and trim parts remain available for a lot of older valve bodies, and replacing a cartridge is much less disruptive than replacing a valve set in tile.
